Understanding
that people diagnosed with Alzheimer’s Disease and
related dementia’s have special needs and require
specialized attention, Concord Health Care Center established
a 29 bed specialized unit.
On this unit, emphasis is on security and safety while
continuing to engage residents in enriching and quality
programming.
The dedicated Activities Assistants on the unit like to
involve every resident in activities, but residents are
not forced to participate. Social activities vary based
on the individual needs of the residents - some residents
are more able to actively participate in social activities
than others. Each resident is given social interaction
by the staff on a daily basis. Activities are offered
seven days a week, beginning in the morning and continuing
into the evening.
Programs and activities are created to maximize each resident’s
independence. We strive to promote overall health and
well-being. A Program Director provides oversight of the
unit and serves as a liaison with family members as well
as conducting on-going assessments. The Program Director
constantly works with the treatment team to coordinate
each individual’s care plan.
